GLP-1 Weight-Loss Injections: A Surprising Boost for Male Fertility?

GLP-1 medications (such as semaglutide and liraglutide) have completely transformed how we approach weight management and type 2 diabetes. Now, new research presented at the ENDO 2026 conference suggests these treatments might carry a surprising additional benefit: improving male fertility and naturally restoring testosterone levels.

A team of researchers from the UK's own University Hospitals Coventry and Warwickshire and Warwick Medical School recently reviewed the data—and the findings are incredibly promising for men struggling with obesity-related low testosterone.

The Myth vs. The Reality

When starting a new long-term medication, a common concern among patients is whether it will negatively impact their reproductive health.

The UK research team analysed multiple clinical trials to see how GLP-1s affected male hormones, sexual function, and testicular health in men aged 18 to 65. The verdict? GLP-1s do not harm male hormones or fertility. In fact, for men carrying excess weight, the medications appear to actually improve reproductive health by tackling the underlying metabolic issues causing the problems in the first place.

What the Science Says

The researchers highlighted several key findings from recent clinical trials:

  • Better Sperm Quality: A 24-week study on semaglutide revealed that while testosterone levels remained stable, patients saw noticeable improvements in both sperm shape and their overall cholesterol levels.

  • Naturally Increased Testosterone: In a 16-week study using liraglutide on men with obesity-related low testosterone, patients experienced a natural increase in testosterone and related hormones.

  • Superior Overall Health: The men in these studies achieved better overall health outcomes using GLP-1s than they did with traditional testosterone replacement therapy alone.

Treating the Root Cause, Not the Symptom

Historically, men with obesity and low testosterone have been prescribed testosterone replacement therapy. However, this new research supports a shift in how doctors treat the condition.

Dr Pratibha Natesh, the endocrinologist leading the Warwick Medical School team, explained:

"This work supports a shift away from prescribing testosterone replacement in men with obesity and low testosterone and toward treating the underlying cause, excess weight and poor metabolic health, which can naturally restore hormone levels and preserve fertility."

By focusing on metabolic health rather than just replacing missing hormones, patients can experience benefits that reach far beyond the weighing scales.

What This Means for Patients

While these findings are exciting, it is important to ground them in clinical reality. If you are considering GLP-1 medications, here are the key takeaways:

  • The benefits are likely indirect: The fertility and hormone improvements are believed to be a direct result of weight loss and improved metabolic health, rather than a magical side effect of the drug itself.
